The Golden Thread of Documentation: Learning the skill of documenting to Medicaid standards

This two-day live training event will provide participants with an in depth understanding of the “golden thread” approach to delivering and documenting Medicaid reimbursable supported housing services.  It will also give participants an opportunity to practice newly learned documentation techniques and receive feedback from presenters.   A key theme will be the importance of starting with a comprehensive assessment that establishes medical necessity, sets the stage for identifying individualized goals and leads to the development of a person-centered rehabilitation plan. The provision of intentional, goal-related services and writing progress notes that link back to the assessment and rehabilitation plan will also be highlighted.
